It is very helpful to have a guide for the documentation in helping the practice transition to ICD-10 from ICD-9. It will also be a useful tool in teaching the physicians the new level of documentation that will be needed for ICD-10.

I always enjoy Evan Gwilliam's presentations. He is very knowledgeable and presents good information. I do take issue with one item in the ICD-10 Documentation webinar. That is the coding in the scenario with "consistent with cervical sprain/strain". The scenario doesn't indicate an inpatient service, so I assume outpatient. Per ICD-9 guidelines and AHA Coding Clinic, "consistent with" is considered an uncertain diagnosis and so is not codable. I suggest those terms be removed from the presentation to make it more accurate. Thanks.
